Established Patient Intake
Already a patient at Aura Eye Center? Use this form to update your information, report any changes in your health or medications, or share new symptoms since your last visit. Keeping your records current ensures we provide the most accurate and safe care possible.
Medical Release Form
Authorize Aura Eye Center to share or request your medical records with another provider. This is required when transferring care, consulting with a specialist, or coordinating with your primary care physician or another eye care provider.
